Em am i the only one that reads that as a fantastic way to say im not sure what i want but if i work it out and its you ill let you know? Apposed to the literal Im dying. Mabye its the poet in me screaming to come up but i think you missed a very cool way of saying just not ready to commit to you yet.

Unfortunately in this case "I have heart cancer" stood for, "I met an easier girl who is into the party scene like me." I ran into him maybe 10 years after he broke up with me, and learned he knocked up the party girl, got married, got divorced, and within a few years had two kids with another woman (whom he was no longer with). Guess he wanted to spread his legacy quickly, before the heart cancer gets him!
