I was hoping he would ask me out but now I don't think it's going to happen. I had lunch with him and another coworker today and found out they had gone to a concert together. And when they met I noticed she gave him a hug and a kiss on the cheek. Should I back off based on these facts? Or should I ask him out before it's too late?

Sounds like he is seeing the other woman. I would not appear too eager or desperate. Ask him out before it's too late?? WTF are you thinking? If he is in to her or they are dating, there is NOTHING you can do. Do not be a fool. Now, if he continues to ask you for lunch, fine, go. You could test the waters on your own and suggest you meet for lunch. IF he's none committal or doesn't take you up on it, then back way off and forget about it. You can still go to lunch if he asks, but do not have any expectations or think that it's going somewhere. DO NOT ask what is going on or what he is up to with the OW. Whatever you do, do not go head-over-heals for him! IMO, you will have to take a back seat right now. He will let you know if he's interested.
