Boyfriend lives in unberable filth

I've been with my boyfriend for 5 months and his lack of cleaning habits are putting a strain on our relationship. He lives with two other roommates and all three of them are slobs. Let me give you some examples of just how disgusting their house is.

1) No one has the ability to take out the garbage. Let me rephrase that, they take out the garbage but they won't put it on the curb for the garbage men to collect. Instead they leave the garbage bags on the porch. They let the bags pile up so there were about 20 - 30 bags of garbage all stacked up on the porch.

2) They don't wash their dishes. One time they let the dishes pile up in the sink for 4 months. Can you imagine the type of filth that was growing in that sink? Last month my bf's roommate got his frat brothers to come over get rid of all those dishes, however, they are starting to stack up again.

3) One of the roommates has a cat and he won't change its litter box for what seems like a week or two at a time. The cat won't use his litter box (can you blame him?) so now he has started going on the floor.

4) The fridge has old moldy food in it. Me and my bf cleaned it out and scrubbed it from top to bottom 2 months ago but now it's starting to get junky again.

5) My bf won't do his laundry. I got so fed up I just started doing it for him. If I didn't, he would wait until he ran out of clothes before he cleaned them.

There's a lot more but just thinking about it depresses me. No one is motivated to clean that damn house except for me. My boyfriend has been getting sick a lot recently and I think it's because of his living conditions. Complaining about the mess has not been working. How can I get my bf to clean up his act?
