The thing is, if I ended up having some kind of powerful unexpected crush on somebody out of the blue, I would permanently remove myself from the situation. This kind of stuff happens. I remember having this irrational "crush" on a cashier at the food store I frequent. It was completely bizarre and seemed uncontrollable and involuntary.

Every once in a while you will meet a person who has this effect on you. Now here's the trick to it all... it's called self control. Are you really going to blow up your marriage for an 80 year old man? If you just avoid him, feelings should subside in time. Find a different doctor.

Out of curiosity, did you experience a love or crush at first sight feeling with this doctor when you first met him?

This is a really unhealthy situation,  you should visit a therapist to help you figure out why you've been obsessed with this man over two decades.
