Storm in a tea cup either way. You care about her because you're worried about hurting her feelings and she cares about you and how you see her. If anything, you can see it as an eye-opener in terms of how insecure she is really feeling.

 

You're right to worry about the real issue, which is her feeling insecure about her appearance - of course, it's her issue to deal with but an honest conversation about actually tackling the issue, and asking her how you can help to sort her issues out; it might just be kind and gentle support or simply being there for her.
