Anywhere, but I have a plastic thing that I slide through the tube as it gets emptied so it feels full all the time.

A t-shirt after removing my sleeping gown and a shawl as it's usually cold.

Middle. I only squeeze the end once the toothpaste runs out and I have to get those last bits of paste stuck in there

I'm usually already dress from the night before in pjs and tshirt so I wouldn't necessarily get dressed so much as getting undressed for my shower.

Just looking into someone's eyes makes me nervous

The middle until the tube is half empty, then squeeze from the bottom only.

Neither. I have no problem looking into someone's eyes when they are talking about how they feel; if I'm talking about how I feel to someone, I want to gauge their reaction, and it's best to look them in the eyes.
