My ex who dumped me twice told me to NC her,she has now sent Bday gift, what do i do?

She knows I love her and yet she sends me stuff on my birthday with xo, love hearts, hugs, on it etc with total disregard to my feelings. It has been a horrible week I have lost sleep, stressed my friends/family out over whether to reply or not and I have judged myself harshly as well. It was cruel of her to screw with my head in order to make herself feel better and she has not even bothered to contact me to see if I actually got the stuff she sent me anyway, I suppose that states her true feelings about me.
