I have a friend who says she has feelings for the guy she is with, and I tell her that if she is in love with him than she will know. To me there is a huge difference between having feelings for someone and being in love with someone. BTW, she has been with this guy for at least 5 years.

Yes, there's a difference between the two.

If she's been with him 5 years, her feelings might not quite be the in-love rush of a newer relationship. But they may be stronger feelings of love and respect, or feelings of affection and familiarity and comfort, or any combination of the above. Or she may have a mixture of positive and negative feelings. Only she can tell you what her feelings mean to her.
