Any who he was out at a hookah bar and when he met some girls he told them his name was Romeo (not really his name, but it does start with an R). Anyways now he's freaking out about how the hell he's gonna tell this one girl his real name when they hang out. Any ideas?

He should have been honest with his name to begin with, then he wouldn't be in the situation. But if he was a "hookah bar", I wouldn't think they would care one way or another.

A hookah is one of those big fancy bongs for smoking tobacco. That caterpiller in Alice in Wonderland smokes one. They typically have multiple hoses to smoke out of. There are some restaurants/bars that offer them with flavored tobacco for people to smoke as a group.

This is no big deal- he could just explain he's not comfortable telling random folks his real name so he uses a nickname from time to time.

I was young and didn't want a boyfriend but wanted to flirt...see if I had the nack to pick up a guy. My middle name is Margaret and I used the nick Maggie. Never giving my first name. Never giving my phone. I can only suppose you (oops I mean your "friend") could come clean with the girl and tell her he never expected to find someone he really felt he could have feelings for. And give up the club. Not a cool place to hang.
