I was wanting to ask someone out, but kinda lack it to do it in person. Is it at all possible or even acceptable to ask her out using a note in her locker? Ive left her various notes in the past, from joke ones to info about whats going on and all that.

You said you used to sent her notes all the time but as jokes and stuff. So she might take the note as a joke. Better to talk to her in person, and actually get a reply.
what is the best way to go about asking her out face to face? Gonna seem kinda hard to just right out ask her.
Do you talk to her alot in person? Find a time when you can be alone with her. Start the conversation with a casual how are you. Proceed to talk casually about school, what projects you guys have, etc, try to gauge her attention so she's focused on you. Don't be overly excited and don't rush through your sentences, act normal, and most all remember to smile and look into her eyes. Casually bring up a movie, like that new superhero movie that's coming out. Ask how she feels about the movie, to see whether she likes it or not, and then lightly bring up say something to the effect that you're interested also, and wouldn't mind seeing it with her. Remember to keep your focus on her, smile, and ask her politely if she would to see the movie with you. If she said yes, you proceed to go into details by asking her when she's free, etc, and remember to ask her for her # so you can get in contact with her.

Things are usually casual or joking, last time we were talking it was about some movie she had recently saw and some minor things. Eye contact is hard for me regardless, but am able to keep a decent focus on her and she is able to keep it strong.

I find eye contact very alluring. It is said a man's constant eye contact on a girl creates the necessary hints to convey his feelings of interest in the other person. Also you can gauge whether she' truly interested or not. A girl who's interested or likes you would return the eye contact but would immediately look away usually because she's shy or has begun to feel a bit flushed in the cheeks. It's the best subtle way of flirting in this world.
You've got good advice already, so I'll just second what's been said. I know exactly how you feel in these situations. I have never been good at them either, but working on the eye contact definitely helps. Probably the single most important thing you can do. Hold it there even after you would normally look away.... and smile. That'll probably come naturally anyway. You have a good start with the easy conversation that takes place already.

They've covered everything but one thing. If you leave a note, she'll take it as a fact that you're extremely shy, or you don't that you don't care at all.

I believe you're in High School.. where notes are popular.. but in this case, let's say you ask her out with a note.. because you're too shy to talk to her.. how do you expect to have an answer?
She will be uncomfortable.. if the 'notes' is not her thing..
Just go straight to her.. with confidence (huge turn-on for girls) and say:
'Wanna f*ck?'... hehehe.. no seriously, just ask her out.. simply.
